How to get my opt-in form in my sidebar

Hey guys,

I'm having some trouble getting my opt-in form into the sidebar on my blog. I've tried using the text widget but that doesn't work.

How could I get an opt-in form like John Chow's? Is there a special widget I need?

Nevermind! I got it!

The plugin over MaxBlogPress did the trick.
