Wordpress Plugin Question

I hope I'm posting this in the right section...I need help.

I want to create a page on my wordpress blog that will allow my visitors to pick multiple items from a list (via radio buttons or whatever) and then after they are through, they fill in their name and any other pertinent information and click submit and it sends the information to me via email OR just holds it in my admin area for me to receive when I login.

Is there a plugin or anything for that? If not, does anyone have any idea how I can do it? A newbie friendly idea...lol

    Looks like you want to build a form. There are several ways to do this, and exsits several plugins in wordpess to do it. But I tell you up front building forms is not something I would call "newbie friendly" ...

    In your dashboard click on plugins and Add New than make a search for "forms" you'll have a good choice there and you could install and try a few, normally the 2 - 3 first ones you see are among the most used. But like I said forms is not the easiest thing to do in web design

    I recommend Formidable Forms at: WordPress › Formidable Forms WordPress Plugins

    It is WordPress 3.0 compatible and will do the things you have indicated, and is fairly newbie friendly to edit.

    It comes with short codes so it is very easy to put on any page or post in your WordPress site & you can have multiple forms.

    I use Contact Form 7 on several of my WP sites - it's easy and reliable and pretty flexible.

    Yes, try contact form seven along with really simple captcha, its really simple to use and interface is very helpful.
