Newsletter Subscription Form

Hello guys,

can anybody advice me to create a newsletter form (just like the one we have hear at the top right) for a health magazine. I would like to embed it in the right side widgets of the site. I have a another form there (which isn't exactly for newsletters. Its a feed subscription form - Check it here - Women?s Fashion Magazine | Health Care Articles | Beauty Tips | Cosmetics and Fashion Information)

Any ideas? is there any free extensions for this purpose? Thank you.
