Wordpress problem with Google DOcs survey

I think a Wordpress pro might know how to solve this problem right away, I am stuck as a Wordpress amateur.

I created a Google Docs survey document (excel), then clicked the Embed button in Google Docs and then pasted the embed HTML code on to a page on my blog, thinking users would see the survey on that page, fill it out, and I could go to Google docs to look at the results.

The end result was, the survey shows up on EVERY page on the blog, it overlays the existing page. I then deleted the page that had the html code, even deleted the Google Doc, and it still shows up on every page on the blog.

Any idea why it is doing this and any ideas how to get rid of it?
