Wordpress theme and plugin for off line lead generation

Hi all, I am building a site to collect leads for a client and I wondered if anyone had advice on:
a) Which theme/colors are best suited for this
b) any good plugins that would be recommended, free or paid, it doesn't matter to me as long as the plugin would significantly increase the number of leads gathered.

    The best ones are very simple and to the point with huge call to action buttons.
