What's the best way to load my opt-in form

I'm having a challenge inserting my opt-in form into index.html squeeze pages for list building. Can someone give step by step instruction or point me in the best direction. I have a hosted domain with cpanel, also have NVU editor.
Do I insert the opt-in before I upload from my desk top? It must be something simple that I'm missing.

    You should be able to insert the opt-in HTML code onto the index.html page of your website. Once you have updated the page, use an FTP program to upload it to your server.

      That's right, but I don't see the form appear on the page. I put in the java scrip in the source area on the bottom bar.
