Wordpress popup contact form?

Anyone know of a good wordpress popup contact form? I'd like to have a button or link on my pages that when clicked pops up an appropriate contact form (based on what page a visitor is on).

Of course I'd love the ability to customize the background of the form and all other aspects of it.

Does anything like this exist for wordpress?

Just to be clear I'm not talking about a popup optin form, but an actual contact form.

  I've tried that one but I find it quite difficult to use and customize.
  
  Also, I think it only allows you to have one form that you use through out your site. So if I wanted different forms to pop up depending on which page a user was on I don't think I could do it.
