please help me to review this site design, the site is simple, give me advice on good theme or template to use to change the layout and appearance.

Just some more css like padding on the edges would help out a lot. Also, why not go to fiverr and get a quick logo made?

Nice and simple. But checking "Turn on file compression" and "Cache images..." doesn't result in any code.

Looks like some "done for you" scripts for $19 or so... tables used for layout...

Of course this is nice. But It will better if you use CSS3 effects to decorate Text-area and Buttons. It will look like Professional. you can see the form on my website to check CBSE SSC Results 2013-14

I'm checking it on iPad and table layout is actually visible. Also the right sidebar ad is too distracting IMO.

I suggest you remove the side bar ad. The ad from the top and bottom will do I think. Nice template and I love the simplicity of it just remove the ad from the side bar.
