Order Form plugin

Hello. Does anyone know of an order form plugin for Wordpress than does not have a traditional shopping cart?

I'm looking for something like this: https://www.shamwow.com/ . Scroll to the bottom of the page.

I want users to be able to just fill out the order form on the bottom of the page without having to adding the product to a cart and then go to a checkout page.

I want something simple, clean and all on one page.

Any ideas??

    May be you don't need a plugin to do that, just creating a page like checkout.php in your worpdress theme folder. Make a payment form in that file ( you can use Paypal form, checkout page or contact via your email...etc ) after that, create a page in wp-admin, point this page to checkout.php. That's way to create a new page template in worppress site.
