Is there a plugin for this?

I have a WordPress site. I want to embed a YouTube video and then have an opt-in right next to it or below it where users can enter their email and name.

I know that OptimizePress will let you do this, but I can't have the video and opt-in taking over the whole page. We've already invested a lot in the design and content of the rest of the page. We just want a sort of mini-landing page at the top.

Is there a plugin that will do this for us?

    Why would you need any plugin for that?

    It's a simple copy/paste "operation".
    Copy/paste the YT embed code and the AR's optin form code. Place it wherever you want...

    You can even post javascript tools without a plugin.
