Does anyone know how to add boxes to a button?

I want to be able to have 2 boxes where people have to enter information which gets sent to me when they pay via paypal like this website :

pagerankbacklinks.net - the people have a place to enter their URL and keywords which gets sent to the guy in the paypal notes bit..

anyone know how to do this
