What's best for surveys?

I have this IM survey for newbies. I have it set up with bold font, and lettering. I have 3 short paragraphs at the top, and then the survey. I don't have any pictures or images.

Should I put some type of graphics, like a header or something?

    I think it's fine the way it is, it's a survey and looks like one.
    only thing i see missing is a privacy policy and blurb about what you will do with my email address
    Might want to include the using of digital delivery systems, design faqs,etc in your manual.
    GREAT idea btw. if you need a minisite for the product when done, let me know, perhaps we can barter something out.

    Good idea BrainDance. I need some type of privacy policy or something. Should it go on the 2nd page, where I ask for the email, or have it on the footer of all 3 pages?

    I created the PHP, HTML & CSS after someone told me to stay away from "3rd party" surveys. I tend to take days to fine-tune CSS, so I wouldn't mind "outsourcing" that on a barter system. I'll keep in touch.

    I just added a 10px green border, and changed the H1 to Impact. I'll post a SS (ScreenShot) in a bit.
