Doubleclick for Publishers - AdSense error

Anyone familiar with Doubleclick for Publishers (DFP)? It is a Google product and the company I work for uses this to place ads within their websites. It requires configuring the graphics, goals, etc. in the Doubleclick backend interface and subsequently configure the <head> and <body> code on the website itself.

We have been running into an issue lately where AdSense ads are appearing in the place of the correct graphics on some page loads. We are confident we have configured the backend settings and frontend code correctly (disabling AdSense integration is as easy as unchecking a box) but this issue persists.

I've searched Google for anyone else experiencing this issue and have not found any other documented reports. Google Doubleclick's troubleshooting and solutions center is of no help.

For some more detailed information, consider the following: After creating a new ad unit and placement (the "places" where the ads should appear on the site) and creating a new order (the "ad" itself) and configuring the order to appear within the correct ad unit, I would visit the page to see if the ad was displaying correctly. On the very first page load, the correct graphic was rendered; on subsequent page loads, there would frequently (maybe 80% of the time) be AdSense ads in its place and only occasionally would the correct ad show when the page was refreshed... After about 20 minutes or more, this issue would stop happening and the correct ad would show 100% of the time. Great! Except there are a few instances where this issue has not corrected itself and we are still experiencing frequent AdSense ads in the place of our own custom-created ads.

Appreciate any insight at all, especially if you have experience with DFP - If I should have posted this in another section, please let me know
