Why has my business profile stopped indexing after being top 3 for over a year?

What have you done in the last year to maintain your position? There are only 3 spots and countless other business' vying for those 3 spots. I can tell you its not an "Update" thing... that at best, is coincidental. The 3 Pack is generally just short of live updating meaning... you could do some things and possibly see the results the same day or the next.

Lets start with the maybe less than obvious...are you checking your listing ( actually physically geographically ) as you always have? Depending on the size of your "Proximity", you might want to take a drive and check with your phone if you are still popping within other areas of your area... or have you lost it all? This type of information will be helpful in assisting you further.

The more obvious things - I would start with adding photos. Have you gone in and looked since your listing became a "Google Business Profile"? I would double check if there are any sections not filled in - that could be. Adding a video will make your listing stronger.

Are there New Physical locations in the same business? Do you sell physical products? are you using the Google business Profile to add prodcts to your Profile? ( https://www.google.com/intl/en_us/re...4aAmeqEALw_wcB )
