Landing Page cretique

Hello all.
I wanted your opinion on the landing page I created:
Prescription 360

What I am looking for is input on design.
- Usability
- Call To Action (What can I do better?)

Here is how I broke it down:
1. Unique Selling Proposition (USP)

The main headline - (Recieve Your Prescription Medications for as Little as $25 a month. )
A supporting headline - (For uninsured Americans Who Cannot Afford Their Prescribed medications. )
A reinforcement statement - (Your Prescription Medications could be as Little as $25 a month! )
A closing argument - (Just fill out our short Web form to begin. With Button-- What are you waiting for? Get started today!

2. The hero shot - (hint at saving money / Hero Image Pills across cash)
3. The benefits - (the visual through text that you could get your prescriptions for as little as $25 a month.)
4. Social proof (I'll have what she's having) - (The Testimonials)
5. A single conversion goal - your Call-To-Action (CTA) (with or without a form)

Any advice / input would be great.

I am basically wanting to get a form completed.

      Can you show me some examples of a page like your are describing. I like the idea.

      They then fillout more info, if they don't you can hit them with a followup on the email kinda.

      Problem with email traps is one complaint and your upstream will pull you if you have anyone saying you didn't get their permission.

        Few thoughts:

        1. Layout-wise, I think it's basically alright. The "get started" button is displayed prominently, the headline is pretty attention getting (though I'd use a bigger font), the visuals are appealing.

        2. I'm not sure what I think about having the website navigation links at the top of a landing page. It seems to provide too much of an incentive to click away from your offer.

        3. The headline could be better. It just seems a little too dry, it's not attention grabbing.

        4. The "easy as one... two... three" thing isn't working for me. I would just do something like this:

        Easy As 1, 2, 3

        1. (Step 1 here) 2. (Step 2 here) 3. (Step 3 here)


        5. I'm also not sure about the "if you are... do not have..." part. Having the headlines and the copy beneath as one sentence really interrupts the flow of the copy.

        Overall though I think this has a lot of potential.

        With a little work it could be a winner.
