Team Lead Capture Pages...How Do I Create Them?

Im currently using "Instabuilder" to create my capture pages in wordpress. How would I make these pages available for my team to import their Aweber code?

Is there a plugin or script for this?

    Companies like Pure Leverage, Empower Network, etc. offer their affiliates pre-made lead capture pages with ".php?id=affiliate id" at the end of the url. And its gerated the moment someone joins.

      Empower Network has been listed numerous times as being crooks. Do a search on them....not good.
