MailChimp + Survey Monkey Integration - Results Tracking

I've integrated survey monkey into an automated email campaign - but not able to see the report in Mailchimp that shows what feedback each contact gave.

I can see the responses through Survey Monkey, but these are anonymous.

When I go to Reports > Analytics 360 (as instructed in MailChimp support articles), I don't see any SurveyMonkey analytics here.

Any one else experience this? What am i doing wrong?
