Wordpress survey plugin with autoresponder

Hey guys, I'm looking for a survey plugin that has an autoresponder integration to it. What do you recommend?

Basically, the objective is to have visitors enter a few questions before they enter their name and email, which is then enter the autoresponder, while I redirect them to my offer.

Anything would help... I couldn't find anything

    Hey Tyronne78, I think I wasn't being clear earlier sorry. I'm seeking a plugin that I can put on my website so that when visitors come, they take a survey and enter their name and email (which will integrate with an autoresponder) in order to reach the destination link.
