Do you know a good script for custom Form creation?

I am looking for a good Form script which can be integrated with a Payment processor and also linked to a database.
Something similar to: BannersMall * not an affiliate link.

Any idea ....

Hi,

I use the PHPform application that comes with cPanel. I use it without databases however when you configure it it does give options to set it up with data bases.

If you have cPanel look under Fantastico and take a look at PHPforms.

George you are right about the Presto form. But i'm looking for something that i can buy and modify. Not a recurring type.
I found a few at Hotscripts but not sure which one is reliable.
