Automatic leads generation

I´m looking for a way to collect leads on a wordpress site and automatically send them to specified clients based on the zip code of the lead.
Preferably if it could be done through mailchimp to utilize their email tracking systems.

Does anyone know of such a "plugin" or can direct me to how this could best be done (preferably in php)?

    Search for optin forms plugin on Google... It will be the first result. You can create optin forms for your sidebar widget and other things. Have a look!

      Hi and thank you for your reply.

      I see that there are a lot of excellent forms out there. My challenge is where do a start... I have a site where people are signing up, and clients who want contact details on only people who are located near them. How do I get contact details on person X to be sent to client Y?
