How Do I Set Up A Facebook Fan Page?

Hey everyone. I want to set up a fan page for Facebook and have a CPA offer for people after they become a fan and tell their friends about it.

I can figure out how to set up that actual page I just don't know how to format it so that they see the become a fan button, then tell their friends, then go to the offer.

Does anyone know how I can do this or can point me somewhere that I can find the info.
