Recommendations on a form generator?

INTERNET MARKETING

I love Google Docs for forms, but I need to make one that can handle pictures.

I'm looking to have screenshots next to a single-line text field.

Does anyone have a recommendation?

Haha yes. Just something like this pForm - Free HTML Form Builder - Create Web Form Template Online that easily accepts pictures alongside the entry forms. If not I'll just try coding it.

  Hey, I never found a good form generator. Maybe dreamweaver has one? But 7 years into webpage development and I am still doing these by hand. Maybe someone will come along with a time saver.
