Best Way To Add Opt in Form To PDFs?

I would be interested to know what methods any Warriors have used to add aweber opt in to PDFs. I am aware of the use of Acrobat Professional are there any other ways? Has anyone tried Open Office Writer and pasted in the code and then created a PDF and got it to work?

    Has anyone ever added an Aweber form to a PDF. Doesn't sound ideal to me.

    I create a capture page and then put the link to the capture page in the PDF.
    The information in the PDF pre-sells and when they go to the capture page, they are going there with the intent of filling out the form.

    I've never seen this done, but I do know that something that may help with this matter a program called Boomerang List Builder. I'm not an expert on it (nor an affilate), but if you're wanting to build a list with PDFs, this seems the way to go.

      You could also possibly embed a Mailto: link in your pdf that lets them send you an opt-in email directly from their own personal email account. See http://www.ianr.unl.edu/internet/mailto.html for the syntax. For whatever it's worth. Maybe also encourage your opt-in-er to add a comment about something relating to your offer.

      I've heard about Boomerang List Builder from asianlunatics. I heard that he is getting free subscribers each day. So it must be good.
