Do all video responses on youtube have to be comfirmed by the owner of the clip you are watching?

    The owner gets to decide whether responses are allowed, and whether they have to be approved. It is a setting for each video.

    The options are:
    -- Yes, allow video responses to be added automatically.
    -- Yes, allow responses after I approve them.
    -- No, don't allow video responses.

    By default, responses are allowed but have to be approved. As with all defaults, many people never bother to change it.
