Insert a hyperlink in the usual way. It depends on the software you are using, if you are using say google Docs. you just highlight the text you want, click on the "Link" button and insert your affiliate link in there (that you have got from clickbank, Amazon etc). You can also add Flyover text (text that appears when you move the cursor over the link text (Anchor Text. Make it keyword relevant for Serps purposes).

When you then create your PDF the link will be in. What software you using for PDF creation?

If you use Google docs you can click File/download/.pdf it will creat the PDF with the links in, flyover etc. You can copy your book to google Docs and create the PDF that way?

I use Nitro PDf and that keeps them, and indeed you can add them to the PDF, but that is paid for software.

I'm using OpenOffice (free office suite software), that produces a pdf with correct links.
Also, instead of putting long url on the pdf, use a url cloaking script installed on your site, and point the links to this script (eg: www.site.com/user/johndoe/index.php?id=124&.... ==> www.yoursite.com/go/promo1).
That could be a great strategy.. if the link change, you must only change the url cloaked, and not the pdf!
