Where the card is being held by a real ahnd and/or real person -- which makes the card look more real + tangible -- is that standard fare for graphics people to do, if requested?

Examples:

Also, how much would I expect to pay for such a graphic? The graphic on the card would have to be superimposed on a stock photo, yes?
