Will pay for help converting a scanned PDF form to a Word doc

I have a scanned bid form for our roofing company, which I've got in a pdf format and I need it converted to Word. I am able to convert it to Word, but I can't type onto the lines where we would normally fill out the estimate for customers. We usually type it onto a template and then print the bid form and it all lines up nicely, but we'd like to do it electronically, i.e. pull up the bid form in Word, type in the info and email it to the customer.

Can anybody do this for me? I will pay you of course.
