All the sites on which I need to monitor traffic are on TypePad, so I use TypePad's built in traffic analyser, which tells me all the above. (I think it's a built-in white-label version of something called "Google Analytics", which is available free).

I like and use Google Analytics. There is reporting within 24 hours, which is fine for me, but might be too long in some situations (such as if you need hourly reports or something similar.) You need a Google account to access the service, the analytics tracking is free. You need to be able to add code to the head area of each page of your website; if you use wordpress there are plugins to help you get Google Analytics up and running. You can track by search engine, referring keyword, top content, plus it gives you stats on where (geographically) your traffic is coming from, as well as what kinds of browsers and internet connections they are using. I'm not affiliated with Google in any way, just happy with the service.

You can sign up at Google Analytics | Official Website.
