Anyone know of anything like that does this? Is it free, does it cost $?

  • I have a questionnaire that I use to collect detailed client information for a personal assessment.

    I'll need something that does multiple choice, yes/no, true/false, comment boxes, etc..
  • The real question is, what do you want the form to do once you hit submit?

    Email you the info? Post to a database? Write to a text file? Integrate into some proprietary application? Add to a auto-responder list?

    IMO answering this question will get you closest to your solution.

  • Got this to install, learning how to use it now.

    Anyone happen to know a good video for designing a custom form using Contact Form 7?

    I need to insert text fields, text areas and drop-down menus. Have 80 total fields to create. Looking for shortcuts...

    • I don't know of a vid but there is extensive online documentation. When you are on the Contact Form 7 page in your WP installation, look in the upper right.
