Where to Place a Squeeze Page?

A little help from all you folks who have experience with squeeze pages would be greatly appreciated.

I have a new web site coming online and I'm going to implement a squeeze page for the first time. But I'm not sure where to place it for the best results.

a. On the index page of the site? (as in www.mysite.com)
b. On an interior page of the site? (as in www.mysite.com/squeeze.html
c. On a subdomain? (as in www.squeeze.mysite.com)
d. On a separate domain from that of the site?
e. As a popup upon loading of your index page?
f. Some place else I haven't mentioned?

My concern about putting a squeeze page on the interior of your site, or using a subdomain, is it seems that folks will just bypass the squeeze when they see your domain structure in the URL. I do that all the time so I don't have to give up my email address.

The problem I see with putting it as the index page is that then returning customers are hit with it upon returning each time. Also, I wonder if the search engines frown on "no content" per se on the index page of your site.

So I'd be interested in any tips or insight Warriors can give who have had experience with squeeze pages. Where does it belong in the site structure?

Also, once the name and email are captured, where do you send the prospect?
