Query about opting in methods

Hi Warriors

I have a blog related to dog training - gundogtrainingblog.com. I've written some posts and created a free report for people to opt in to my list. Even though I have like 30-50 views per day ( most of the traffic generated through my signature on related forums) I only got 1 member - blog is up for about 2 months now :/
Please tell me whats wrong with my opting in techniques because obviously I must be doing something wrong.
All comments would be appriciated.

    I had to hunt for your form. Put it right at the top of the sidebar. I would also put an opt in at the bottom of each post (before the comment section).

    Your copy above the opt-in isnt very enticing either. If you make it more exciting people will love to get it, for example:

    "Train Your Hunting Dog In 5 Easy Steps With My Simple Guide - Just tell me where to send it" make the first stand out more with bigger text, then make the submit button say "Free Download" or "Free Guide"

    If you're getting that much traffic you should get a LOT more optins with those changes I hope you do. PM me the results next week!

    If your goal is to get the maximum number of site visitors
    joining your list, then send them to a squeeze page instead
    of directly to your blog.

    At the squeeze page, they only have 2 options: to sign-up
    your list or to leave. Simple.

    At the moment, on your blog there are too many options
    that a visitor could choose from (e.g. to read your latest
    post, read recent posts, read recent comments, etc).

    Plus, if you want people to sign-up to your list from your
    blog then at least place your damn opt-in form at the top
    of your page so they can see it without scrolling.

    Consider putting your opt-in form in your site header or
    in your sidebar. You could also use an entry or exit pop-up.

    Plus, you need to make your opt-in form jump off the page
    so it's the first thing they see when coming to your site.

    Make them an irresistable offer so that they really, really,
    really, want to join your list. What information would be
    really valuable to them?

    You could try sending your traffic directly to a squeeze page where there is nothing but your free report.

    Another option is to make some kind of "popup" when people enter the site where you
    ask if they want your free report.

    As it is right now the free report isn't easy to see and therefore most people simply wont see it. You could try a different position or make it bigger.

    Just my thoughts, hope some of it gives you an idea

    EDIT: Looks like someone already suggested most of it while I was writing :p
