You dont really need a wordpress designer then... I personally use Michael Ottman at simpleecovers.com. He has various different packages that would probably suit you. Or you can look at elance, fiverr.com, odesk or even the warrors for hire section in here.

Im sure someone will join this thread that can do it all for you though

  Agreed - I've used Karl for lots of jobs and he's always done a great job, everything from wordpress themes, physical book cover graphics to cartoon images.

It is always difficult to find the right provider if you have no idea what you need.

For a header image and other graphics - you need a graphic designer.

To make a WP theme - you need a good graphic designer and a good theme developer/coder. (Rarely happens to be the same person...)

Check out odesk, freelance etc. Possibly even worth trying fiverr or zeerk or one of those type of sites. Always ask them to show you previous work, it will help give you an idea if its the kind of design you are looking for.
