Help me solve my number 1 biggest problem : the SQUEEZE PAGE !

For 3 years I am struggling with this problem.
I procrastinated, have been terrified by it, so I tried very little,
and always got stucked at the 1st smallest step.
I have icontact and the signup forms they provide for squeeze pages
are just terribly ugly.
I want a big big field for email that noone can miss, like optimizepress is doing (however I can't afford now $97 for optimize press).
My problems which I want to solve (and I will be grateful if you can guide me):
1.How to make an attractive big and nice signup form,
with big big field for email input and very good looking ?
2.How to input the icontact code for using icontact
with my list? The signup forms that icontact provides are
so ugly that noone can really use them.
3.Do you know ANY simple or pre-made GOOD LOOKING squeeze pages
that I can use and just paste... what part from icontact code
in order to build my list?
I will be very grateful to everyone who can share their insights and ideas here.
