I have a wp template for a squeeze page -- it can, of course, enable me to insert the aweber optin box code.

In my case, I do not want to build a list, I just want visitors to email me to ask for more info (one time).

My question is, can I insert a box that just has them enter their name, email, + phone #, + a comment box?

Is that just as easy as putting the aweber code in?

If so, how do I do it? (sorry if this is a dumb question).
