I want to put a WP quote request form in my sidebar.

Nothing fancy. Name, phone, email, and a couple check boxes.

The plug-ins I've been finding are pre-made contact forms, which either don't fit in the widget area, or don't allow me to add the check boxes.

Thoughts?
