Sidebar Optin Form Plugin w/ Templates?

I've been looking but can't seem to locate a Wordpress Plugin that syncs with your auto responder service that creates a sidebar optin widget with attractive templates or customizable templates.

I'm looking to create something attractive. Things like...

Only thing I've found close to what I need is Sidebar domination that is a bonus product to purchasing Popup Domination. But all I need is the sidebar widget and it's not sold separately. Anyone know of anything similar?

Frank Kern had a set of wordpress templates that worked like that. It came in one of this big $1997 products. Mast Control or List Control. One of those. Try to do a search specifically in the war room, there may be a template like that for free.

I do full width header, sidebar and post footer optin forms. I am working on getting a plugin created for a less custom/more commercial appeal and price point.

Optin Skin is also a good product, though.
