Question on Implementing Mike Dillards TF 2.0 system

Maybe I missed it somewhere, but Mike Dillard (in MLM Traffic Formula 2.0) is talking about the importance of following up by calling leads immediately (which I totally agree with).
The problem is that I'm missing where it is that he's asking for a phone number from leads.
In his example squeeze page, he gets a name/email to put them into the auto responder... is he getting the phone after the initial product purchase? or is there a special page that readers are sent to that requests the info?
Somewhere I'm missing this, and it seems so basic.
Also when directing leads that are talked to on the phone, Mike mentions sending them to a business site. Is this the MLM replicated site, or is this a site we've created that improves upon the replicated site?
