Anybody know of a good web form software?

Looking for a robust software that can create better looking web forms then what Dreamweaver has been giving me. To be super hopeful, it would be great to find something that could produce a drop down type form that neatly hides itself away until the user decides to click on it and fill it out. Maybe an accordion style form? If that even exists?

CSS or HTML based doesn't matter as I am sure I can make either work. I just feel there has got to be something out there that can produce the external code that I can input into my page.

Search has produced nothing but the paid hosted type web form service that I am not really interested in. Besides, a lot of those services seem to be behind the times as far as how their forms look unless I want to pay a big monthly payment. Would rather pay a one time price for a software that outputs code. Searching here on the forum didn't produce anything for me either, perhaps I am using the wrong keywords though.

If anybody knows of a software program close to what I'm looking for, I would greatly appreciate a link. Thanks in advance!
