Opt In Form at the bottom of monitor - what's it called?

sorry if this question has been asked before. I was wondering whether someone can point me to the name of the plugin which will allow me to add an opt in form at the bottom of the screen?
I am not asking for something which is available at the end of every post, but rather at the footer of the visible screen and which autoscrolls when the visitor scrolls down or up.
