Questionare to PDF Output - Anyone?

Before I saddle myself with writing a system that asks a series of questions and then at the end, outputs a PDF based on (and including) those answers.

Anyone have any clue what I am getting at here?
Does not need to be a WP plugin but sure would be nice.

PHP/MySQL I am looking at hotscripts but cant get the right wording for searching this script out.

    Depending on what exactly you're trying to do, I think that Survey Monkey may be a good solution.

      Oh build a page, based on user input, that includes their name and address, the choices they make in the survey determine the verbiage that will eventually be output.

      Gots to be PDF so checking your monkey-mention, thanks for that.

    You can use any system that returns the answers in HTML format, then use a service like HTML to PDF | HTML to Excel | DocRaptor to convert that HTML to PDF. We use DocRaptor to generate PDFs at my company...have tried several services and free plugins, and they are the best. No affiliation except as a satisfied customer.

    There are a couple tutorials floating around where you can use domPDF and swift mailer to scrape form data and populate a PDF and then attach it to an email. Its written in PHP and HTML. Here is one of the original tutorials.
