Question about DMCA

I have a question about DMCA.
If some person give me all information like his name, phone number etc can I report infringement on his behalf?
For example, if I setup DMCA generator like this one: DMCA Notice Generator and Sender do I need to write something on the page like terms of use? in fact, they send a report to the infringing site on the behalf of original author and this service are not an authorized person or something else.. :confused:

      I don't know but my idea is more cooler than that, so I need the answer for my question and maybe I'll present some revolutionary service in the near future

    It's a no brainer that you would need a terms of use for a service like that.

    There are severe legal and financial consequences for sending a false DMCA notice.

    As far as I see, that form sends the report to the infringing site. How effective is that? Would an infringing site care about your report? That report has to go to the hosting provider. That form is pretty useless (unless it is sent to a file sharing site).
