How do you make an online quiz to capture leads

INTERNET MARKETING

I want to make an online quiz where it gives people customized solutions based on their results. Here are examples of tests I am looking at. I am using wordpress btw.

or this guy who does the opposite where you first have to give your email before you can take the test

Any ideas on how I can get this setup? Is this a plugin? or some customized solution?

  A very similar WP plugin was just introduced by Shane Melaugh: Viral Quiz Builder. His initial discount puts the price close to Valentin Jed's Laser Focused Leads. It's not on Warrior Forum or anywhere but his own page.
