Hello, I'm looking for wordpress plugin (free or paid) for contact form which I can embed the affliate id in the form. So, when someone fill in the form, the details of the person/leads will be emailed to the affiliates.
The website url reflects the affliate id: http://website.com/?id=12334, 12334 is affliate id.

    You can use contact form 7. its does pretty much all the job you mentioned, just set your receiving mail id to your affiliate vendors mail id. Its free too .
