WP Contact/Free Quote Tab

Can someone recommend a plugin for WP for this? I seen them in the past on a few sites.

Usually on the lower right of the screen will be a tab that sticks up that says contact us or receive a free quote. Not live support type.

Trying to find something that a user can click on tab, it expands open and either enter phone number and email and have it send it to me. Or just have it go to a different page if need be.

Any suggestions?

    Actually found something called "Sideoffer" plugin that appears to do what I need and place a form in it. So if anyone has used something similar please let me know and suggest.

    EDITED:
    I cant use the above. When the site is in responsive on a small screen the tab doesn't reduce in size.
    Defiantly open to ideas.
