How do you do publishing notes?

The publishing notes that you get at the beginning of books (i.e. copyright, etc), how do you do them? Is there a standard thing that you write, or how does it work? Is there a template that you can use?

    If you don't have a template, I would just copy(when writing using Open Office, or Word) what is written in a published ebook. Write down the copyright information, all rights reserved, etc. just as you see it-whatever it is you want to include in your book. Here's a website that gives a sample.     Most times, you can simply copy the copyright notice or disclaimer from another similar book. Assuming you are talking about ebooks, this information would go into your swipe file for use on all digital ebooks.
