I'm filling in all the tax questions, and there's one that I'm not sure about (I'm on the last page):



I'm spanish, so I don't use my 1st and 3rd names.

For example :

John Edgar Smith = John Smith (in the UK or US)

But I would be : John Edgar. I don't use my 3rd name, ever.

My bank account name is All my names. i.e. MR JOHN EDGAR SMITH

So what do I put? I'm on the last page, where "under penalty of perjury" I have to confirn that everything is, to the best of my knowledge, correct.

That's why I'm confused. They've got me down as John E. Smith (not my real name, just an example). But I'm actually John Edgar. There was no other option but to put down John E. Smith. The form didn't allow any other way.

And now it's asking me what capacity I'm acting in. There's a huge drop down menu with CEO, Treasurer, etc etc etc. What do I put?

And when I sign, do I do it in all caps?

    Sounds like you are over complicating the process. If you are not sure, check with a tax accountant, and refer to legal help.

    Personally I wouldn't really worry about this too much.

    Use the name on your bank account, or the name under which you file taxes.

    Sign with that name, all caps or standard case.

    There is a phone number on the IRS page for this purpose. I was able to sort my EIN number this way and they gave the number to me on the phone. I received it in the post 3 weeks later.
