They are already available on Fiver actually
So not that new..

{This is a new method for honoring the memory of your loved ones. You will receive a digital memorial page with 1yr hosting that will contain an image of your loved one, DOB/DOD, bible verse/poem/obituary up to 500 words. You will also get a QR code that when scanned with a smartphone, will direct users to the digital memorial. This QR code can be printed along with the newspaper obituary or made into a plaque/sign and placed at the grave site.}

Actually, there are people doing this in the US putting QR on headstones...I saw a story on TV about it, but could not find a link anywhere...

At first my reaction was that this example us a bit distasteful but it really does make sense. Great thinking!
