Referrals and taxes

I am considering offering the opportunity for others to refer me to people they know. I would then give them a set fee for each paid client.

Here is my question: Are there forms that should be filled out prior?
I know for example some of the affiliate sites require you to fill out a form online.

Can anyone tell me what should be filled out prior? I know that you need their social security number for taxes.

I want to head down the right road here.

Keith, I don't live in the US and I don't know how things work there but l think you can't get a certain answer to your question on this forum. Any replies will not be more than a comment, a recommendation or a suggestion. It would be best to ask a local accountant who is experienced in things you are doing so that you can get a clear answer.
