Google Places & directories fill-in sheet

(Sorry, I'm a bit tired and can't think of a very good title!)

I want to develop a simple sheet that businesses will fill out in order to provide me with most of the information I need to submit their business to Google Places, Yelp, CitySearch, and the like. Here is what I have so far...

Company Name
Street Address
City, State, ZIP
Main Phone #
Fax # (if applicable)
Email Address
Description (200 characters max)
Business Hours
Payment options (cash, check, credit card, PayPal, etc.)
Do you have photos (logos, interior or exterior photos), or would you like us to take these photos?

I am leaving out categories because I should be able to do that myself. Are there any others I should include?

    If you go into google places, you'll see the additional options they display and you can make up some of your own.

    Some businesses will have info that others do not.

    For example, a restaurant I just did, we listed parking, hd tv's, and some other details pertinent to that particular place.

    Personally, I wouldn't just hand a sheet over - but actually talk the client through every detail that I could - as I believe it is those tiny details that make the difference in the listing.

    it really would depend on what yelp,google places wants. I mean there is the basic info but other things vary on provider/ what industry your in.

    I would take screenshots of the forms from the different sites and compare/contrast what they want. Then just create a .pdf form from that.

    Might check with ubl.org, they do listings for businesses all over the place and I'll bet their sign up form is very complete. You might even want to outsource the deal to them and pocket a nice bit of change for very little work.

    Something else you could consider is using Google docs to make a form the biz owners could fill out. The results are automatically entered into a spread sheet all neat and tidy for your use.
