Catching a ride on a pizza box

After thinking of how to drive traffic to web video commercials for local businesses, I started thinking of targeting pizza shops.
Then it hit me.
I'm sure a decent (or even struggling) pizza shop will send out hundreds of pizzas in their boxes each week. How about attaching a small flyer (maybe 1/4 page of an 8.5x11 sheet to keep the cost down) which mentions a "free appetizer" or something like that.
This "free appetizer" flyer would include a website (landing page) that customers would go to, enter their name and email address, and be sent a coupon. It would then sign them of for a "Deal of the Week" campaign offering other coupons once they subscribe.
That would be an easy way for a pizza shop to start building a list that they can market to.
With hundreds or more boxes being sent out every week, I'm sure the list would grow quickly.
To sweeten the idea, there are a lot of pizza shops in my area that also have full restaurants. So this could turn a "take out" customer into a "dine in" customer as well depending on the coupons offered.
Now, if you think that's a good idea, how could we as offline marketers capitalize on it?
If the pizza shop wanted to give it a try, how would you charge for it? A certain amount per new lead? A standard monthly maintenance fee?
I'm just throwing it out there to see what you all think.
